Surgical techniques for IOL fixation without capsular support have rapidly improved in the past few years. Agarwal’s “Glued IOL” and the Yamane technique have become popular methods for secure fixation of 3-piece IOLs. SPA toric and MFIOLs have not been amenable to these methods. Using the Double Flange Technique we demonstrate secure fixation of these IOLs with proper centration and alignment.
